Target Population:
Severely mentally ill adults. Criteria includes: 120+ days homeless, 30 days in a Psychiatric Hospital, 6 months in an IMD, 30 days in jail, in the last year.
Ages of Population:
Adults aged 26-59 who have a major (Axis I) mental health diagnosis, and/or dual diagnosis, and who are in need of intensive support services.
