Ethics & Compliance Reporting
San Fernando Valley Community Mental Health Center, Inc. is committed to fostering a culture of integrity, accountability, respect, and ethical decision-making. The Ethics & Compliance Reporting System provides a means for individuals to report concerns regarding suspected misconduct, unethical behavior, policy violations, fraud, waste, abuse, safety concerns, or other matters that may impact the integrity of the organization.
This is not an emergency reporting system. Do not use the Ethics & Compliance Reporting System to report an immediate threat to life, safety, or property. If you require emergency assistance, call 911.
Who Can Submit a Report?
Reports may be submitted by employees, clients, vendors, contractors, volunteers, and community members.
What Can Be Reported?
Examples of concerns that may be reported include:
- Fraud, waste, or abuse
- Policy violations
- Unethical conduct
- Harassment or discrimination
- Retaliation
- Safety or security concerns
- Other compliance-related matters
Confidential and Anonymous Reporting
Reports may be submitted confidentially or anonymously. Individuals are encouraged to provide as much information as possible to assist with review and follow-up; however, providing your name is not required.
All reports are handled as confidentially as possible and shared only with those who have a legitimate need to know in order to review and address the concern. Reports will be reviewed and handled appropriately and in a timely manner.
Protection from Retaliation
San Fernando Valley Community Mental Health Center, Inc. prohibits retaliation against any individual who, in good faith, reports a concern or participates in an investigation.
